Our Nurse-Led Wellness Program, guided by the knowledgeable and compassionate Nurse Becca, offers a holistic approach to support mind, body, and spirit starting in your kitchen.
This is not a medical membership. It is a wellness-focused program designed to complement care through lifestyle measures.
1. Comprehensive Intake and Evaluation: Initial intake includes detailed lifestyle evaluation, laboratory testing, and body composition analysis (skeletal muscle mass, percent body fat, basal metabolic rate and more)
2. Holistic Wellness Support: Tools and strategies to create a healthier nourishment culture at home. Nurse Becca offers personalized guidance on sustainable changes in diet, movement, and daily habits.
3. Management of Nourishment-Related Conditions: Nurse Becca works in collaboration with Dr. Adame to provide personalized lifestyle-based interventions .

As parents of recent Culver grads, Dr. Adame and Nurse Becca understand the unique challenges and pace of Culver boarding school life. While we maintain a collegial, collaborative relationship with Culver Academies and the Health Center, our priority is your student.
1. Easy Enrollment: Culver Academies students can enroll immediately, without an inquiry session.
2. Comprehensive, Student-Focused Care: From acute illness and mental health to ADHD, sports injuries, Accutane management, and sports physicals, we specialize in the health needs of boarding school students.
3. All-Inclusive Membership: $250 monthly membership fee covers all services at Culver Pediatrics Center for your peace of mind.

PANS (Pediatric Acute-onset Neuropsychiatric Syndrome) and PANDAS (Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal infections) are conditions that result in a sudden and dramatic onset of neuropsychiatric symptoms. Dr. Adame is an expert in the treatment of PANDAS/PANS and other complex conditions.

We accept patients with and without insurance. We do not bill insurance for our services as we operate on a Direct Primary Care membership model. We work directly for our patients, not insurance companies. However, we strongly encourage families to maintain their insurance coverage for services outside of our practice, such as specialist visits, hospitalizations, and emergency care.
Direct Primary Care (DPC) is an innovative, membership-based healthcare model.
Instead of billing insurance, patients pay a flat monthly fee. This gives us the freedom to provide comprehensive pediatric services with a focus on personalized care, longer appointment times, and direct access to our team. Our goal is to make your child's healthcare straightforward, accessible, and centered around their needs.
Our DPC Membership Services
Our comprehensive membership includes a wide range of services to keep your child healthy and thriving:
Well-Child Visits & Sick Visits: Routine check-ups and same-day or next-day appointments when your child is ill.
Immunizations: Keeping your child's vaccinations up-to-date.
Developmental Screenings: Monitoring your child's growth and development.
Chronic Condition Management: Ongoing care for conditions like asthma, allergies, digestive health, mental health and more under the expert care of Dr. Adame.
Nutrition Counseling: Guidance to help your child build healthy eating habits.
Adolescent Care: Specialized care for the unique needs of teenagers and young adults.












Scheduling an Appointment: We use SigmaMD for our electronic health record platform. You can easily self-schedule appointments on the SigmaMD app once you enroll and download the app on your phone.
Telemedicine Visits & Phone Consultations: For your convenience, we offer telemedicine visits and phone consultations for some visits types. Simple, uncomplicated sick visits, chronic care follow ups (including most PANDAS/PANS Follow Up), and the initial patient intake appointments can be done by Telehealth. Get the care your child needs from the comfort of your home.
After-Hours Concerns: Upon enrollment and activating your child's membership, you will be able to send our team messages through the SigmaMD app. You can send a chat message to Dr. Adame directly for after-hours concerns. She typically responds within a few hours. For urgent emergencies, please visit the nearest emergency room.

Ages Treated: We provide dedicated care for newborns through young adults up to the age of 22. Most of our families keep their membership with us through the end of college graduation.
Your Child's First Visit: To ensure a smooth and meaningful first appointment, we kindly ask that you submit your child’s medical records and complete the intake forms at least two weeks prior to scheduling the initial visit. This helps us prepare thoughtfully and make the most of our time together.
Prescription Refills: Prescription refill requests can be easily made by sending us a message on the SigmaMD app refill chat feature. Refill requests take us 48-72 hours to process.
Care for Out-of-State Patients: We proudly care for families from across the country. To establish care, your child must be seen in person at our Culver clinic within a few weeks of enrollment. This visit is essential to meet legal requirements, perform a baseline physical exam, and ensure safe, personalized care. Follow-up visits are often available virtually. For families unable to travel, a doctor-to-doctor consult may be an option.
No Minimum Contracts. Cancel membership any time: You can cancel at any time. We understand that circumstances change, and we will help make the transition as smooth as possible, including sending your child’s medical records to their new doctor upon written request.
If you decide to return to Culver Pediatrics Center in the future, a re-enrollment fee of $1,000 per child will apply to reactivate membership, which helps us manage the resources needed to provide personalized care to each of our patients.
Care Made Simple with the SigmaMD App: We use the SigmaMD Patient App to give you seamless access, convenience, and high‑quality care at your fingertips. You can message our team, view all your child’s health records (labs, clinical notes, prescriptions, etc.), schedule appointments easily in a few taps, and securely handle billing and payments—all in one place.
IVIG Treatment Information For PANDAS/PANS: If IVIG is indicated, Dr. Adame typically sends the orders to Memorial Beacon Outpatient Infusion Center, about 50 minutes north of Culver Pediatrics. They provide excellent care, and we work closely with their team.
IVIG is a two-day outpatient treatment. If you're not near South Bend, IN, we can help find a closer center. Keep in mind Dr. Adame is only licensed in Indiana and Ohio, and some locations require an order from an in-state physician.
We’ll submit a prior authorization (PA) to your insurance, though processing times vary widely. While we’ll advocate and appeal as needed, coverage decisions are ultimately made by your insurer.

Vaccines: We offer routine childhood immunizations and flu clinics for all ages to members and non-members.
Medical-Grade Ear Piercing: A safe and professional option for your child. We use the Blomdahl Medical Grade Ear Piercing system.
InBody Scans: A detailed analysis of body composition to support health and wellness goals.
