We offer laboratory services to the community. No need to be a member. Doctor’s Order Required.
$25 Lab Draw Fee Plus the Cost of Labs
Follow 4 Steps to get your labs done with us:
1) Email Nurse Becca nurse@culverpediatrics.com or call 574-544-2343 to schedule an appointment.
2) Ask your doctor to fax the lab orders to our clinic at 574-544-2339. Or you can email Nurse Becca a copy of the order prior to your appointment. We will fax your results to your doctor.
3) Complete this consent form prior to your appointment. In this consent, you will designate if you wish to pay for the cost of labs through insurance or use our deeply-discounted self pay prices.
4) Pay the $25 lab draw fee by clicking the purple Add to Cart button below. This fee is separate from the cost of labs.
PLEASE NOTE: We do not offer medical advice or treatment for abnormal lab results to non-members.
The $25 lab draw convenience fee is not subject to insurance billing and is due before the service is rendered. Show receipt of payment on the day of your appointment.

If you choose to use your insurance, we make no guarantees about coverage, out-of-pocket costs, or co-pays. If you choose to use your insurance, please direct any questions about billing and fees to your insurance company.
